Step 2 • Application

IMPORTANT NOTICE: Your Letter of Inquiry (step 1) must be submitted and approved before submitting the Application.

If the Foundation accepts your organization's letter of inquiry, you must complete and submit the Application Form by the May 15th or November 15th deadlines. Application Forms may be viewed and downloaded by clicking on the link below and saving the file to your computer. Please do not alter the Application Form.

Three copies of the application and materials must be received by 5:00 p.m. CST on May 15th or November 15th of each calendar year. Should the application deadline fall on a weekend or national holiday, the application is due the following business day.
